: Ethical approval was obtained from the Berkshire B Research Ethics Committee (REC) in the form of a substantive amendment to the GRASP trial (REC Ref. 16/SC/0508; Integrated Research Application System (IRAS) ID 199243). The GRASP trial was approved by the UK Competent Authority, the Medicines and Healthcare Regulatory Agency (MHRA) (EudraCT number 2016-002991-28) and has been registered on the ISRCTN clinical trial register (ISRCTN number 16539266); the PROMPTS SWAT was registered as a sub-study of GRASP on the ISRCTN register.
