: The trial has received ethical approval from the Medical Ethics Review Committee (MERC) at Maastricht University Medical Centre (MUMC), the Netherlands (reference: NL64393.068.17) at August 1, 2018. Written informed consent will be obtained from all participants before inclusion in the study. A translated version of the informed consent document can be found in the supplementary materials. The Clinical Trial Center Maastricht, which is independent from the study sponsor (i.e., Maastricht University), will conduct trial-related monitoring, audits and MERC review.
