: The trial has received a favourable ethical opinion from the London-Dulwich Research Ethics Committee (REC) (REC Reference: 18/LO/0362 IRAS: 238744), both centrally and locally for all sites involved in the study. Informed consent will be obtained from all study participants and/or their guardians if under 18 years old for the cost-effectiveness and process evaluation studies. No consent is required for the main trial outcome, which uses only anonymised and routinely collected case note and outcome data.
