: The study was approved by the Medical Ethics Committee (Medical Ethics Committee United) on 30 November 2020 (NL74744.100.20) []. The Maasstad Hospital board of directors approved the study on 7 December 2020 (L2020108). The Spijkenisse Medical Centre board of directors approved the study on 28 September 2020. All patients will have to provide written informed consent before participation. At the referring hospitals (EMC, FGV, LUMC, ASH), patients will not sign informed consent forms and no study procedures will be performed. Therefore, they require no approval from local boards of directors. Patients are referred on the basis of clinical expertise. Collaboration with the referring centres in this way has been approved by the MEC.
