: The study protocol has been approved by the Children’s Health Queensland Human Research Ethics Committee (HREC) (HREC/16/QRCH/81), New Zealand Health and Disability Ethics Committees (17/NTA/120), Cantonal Ethics Committee, Zurich (2021–02281) and Canadian University of British Columbia Children’s and Women’s Research Ethics Board (H19-01636). This study has approval for a consent-to-continue (delayed consent) model for participating sites where allowable under local legislation and governance processes.
