: Ethics approval for this study has been granted by the South Western Sydney Local Health District Human Research Ethics Committee (AU/1/970A317) in Australia (see Additional file InternalRef removed) and the Northern B Health and Disability Ethics Committee (19/NTB/133) in New Zealand (See Additional file InternalRef removed). We confirm that all methods were performed in accordance with the relevant guidelines and regulations. This study did not include the use of tissue samples. All informed consent forms have been approved by a responsible Independent Ethics Committee with jurisdiction for the participating site prior to the beginning of the trial at each site. All eligible participants will be presented with the participant information sheet and consent form outlining the purpose and content of the survey. Written informed consent will be obtained from all participants prior to the commencement of the research activities. They will be informed that they can withdraw from the study at any time.
