: This study was approved by ASU IRB (#STUDY00017678). During in-person interview, the decisional capacity of participants is assessed by trained study staff using the University of California, San Diego Brief Assessment of Capacity to Consent. Participants who demonstrate capacity consented for themselves, while those who lack capacity provide assent and the surrogates provide surrogate consent. HIPAA is obtained similarly.
: Since modifications to the consent form can occur during the conduct of the trial, we will not provide a model consent form at this time. However, we are willing to give our final version of the consent form after the study concludes.
