: ACURE4Moms has been approved by the University of North Carolina Institutional Review Board (IRB) as of August 30, 2022 (Review Number 22–1541). Written informed consent will be obtained from all participants in the longitudinal patient survey and for the participants in the implementation interviews. A waiver of informed consent was obtained for obtaining health information exchange authority and vital records data on pregnancy outcomes for participants given that the interventions are delivered at the level of the practice, focused on quality improvement, and pose minimal risk to participants. However, data security contracts were set up between each of the participating practices and UNC to and between UNC and the Health Information Exchange Authority to clarify how data would be stored and protected. Protocol deviations are reported to our sponsor, our independent safety monitor and the IRB.
