Written, informed consent will be obtained from all adult patients and the parents or guardians of patients 14–17 years of age. Assent will be obtained from patients 14–17 years of age. Informed consent forms were approved by the Ethical Review Committee, Faculty of Medicine, University of Ruhuna, IRB-Duke University, SLCTR, and ET&R and will be used in the trial.
