: This article includes a secondary analysis of the Healthy Start, Happy Start trial (HSHS) SWAT which was embedded within the HSHS trial. The HSHS trial was pre-registered ISRCTN58327365 and received ethical approval from the NHS London–Riverside Research Ethics Committee (Ref no: 14/LO/2071). All patients provided informed consent. The embedded HSHS SWAT received ethical approval as a substantial amendment (dated 13 January 2017) to the trial.
