: The DHS protocols and procedures were reviewed and approved by the institutional review board (IRB) in each of the host countries and the IRB of ICF Macro, and the Centers for Disease Control and Prevention (CDC) in Atlanta, GA, USA. Prior to the interviews and blood sample collection, all participants provided written informed consent. Permission was granted by the DHS Program for the use of the data. An IRB of ICF Macro ensured that the surveys were implemented in line with the US Department of Health and Human Services regulations for the protection of human subjects—Code of Federal Regulations Title 45: Public Welfare, part 46 (45 CFR 46) [CitationRef removed], while the host country IRB ensured that the survey was executed in line with the local laws and norms.
