: Ethics approval was given by the Regional Ethical Review Board in Stockholm, Sweden (ethics permit numbers 2008-351-31, 2012/2106-31/4, 2012/222-31/3, 2014/995-32, and 2015/416-31). This ethics permit allows for use of clinical samples for analysis of scientific importance as part of clinical development, and for lifting clinical filters to interrogate the whole genome in selected cases.
