: Institutional review board (IRB) approval was granted by the Massachusetts Institute of Technology Committee on the Use of Humans as Experimental Subjects. Samples were collected under study approvals of the IRB committees of the participating institutions: Mass General Brigham (covering both MGH and BWH); Beth Israel Deaconess Medical Center, Boston; and University of California, Irvine (IRB approval numbers 2012P001062, 2012P000205, and 2012-8957, respectively). Waivers of informed consent were granted for each institution due to the fact that bacterial isolates were collected for the study after all clinical work was completed, including after some patients were discharged, which made obtaining informed consent logistically infeasible.
