: The Cape Town study was approved by the Health Research Ethics Committee (HREC) of Stellenbosch University [N16/03/033 and N16/03/033A] and the Faculty of Health Sciences Human Research Ethics Committee of the University of Cape Town [HREC 755/2016 and 702/2017].
