: This study project was performed respecting the principles of the Declaration of Helsinki. The study of the German cohort was conducted in accordance with the approval from the Ethics Committee of the Medical Faculty of Kiel University and complies with the rules in the General Data Protection Regulation. All participants gave written informed consent to participate in this study.For the Danish cohort, written informed consents were obtained from all participants.
