Ethics approval and consent to participate
: This study was approved by Ethical Review Board (ERB) of Nepal Health Research Council (NHRC), Kathmandu, Nepal (Reg. No. 494/2018). Written consent was applicable to literate people while verbal consent was approached for the rest subjects. Parents/Guardians were interviewed in case of children.
