: The study protocol was approved by the ethics committee of the Central Finland Healthcare District (U8/2017) and was registered prospectively in the International Standard Randomized Controlled Trial Registry (ISRCTN69044459). All participants aged ≥ 18 years and the legal guardians of the underaged participants signed written informed consent. Participants < 18 years signed written informed assent assent before participation. Reporting of the present study follows the CONSORT guidelines as applicable to a non-randomized clinical trial.
