Ethics approval and consent to participate
: All procedures performed in studies involving human participants were in accordance with the ethical standards of the University of Northern Iowa (no protocol number), Miami University (#05-060), and with the 1964 Helsinki declaration and its later amendments or comparable ethical standards. Informed consent was obtained verbally or in written from all individual participants included in the study. Due to data collection that necessitated the use of telephone interviews for those who were geographically distant from the research team, the Institutional Review Boards of both universities approved the use of verbal consent in cases where obtaining written consent was not possible. All participants received a copy of the consent information prior to and again at the time of/immediately following the interview.
