Ethics and personal protection
: The present study was approved by the physicians at Risvollan health center, the Norwegian Centre for Research Data (NSD) and the Regional Committee for Medical and Health Research in Mid-Norway. Each still living patient (n = 67) who was included received a personal letter with information on the study, and information about the right to withdraw from the study. The physician who manually registered data in the medical records (MH), was released from the patient confidentiality by the Regional Committee for Medical and Health Research.
