: Ethics approval was obtained by The King’s University Research Ethics Board (#2016-55-LW), the University of Alberta Research Ethics Board (Pro00069640); Alberta Health Services, and NACTRAC provided operational approval for conducting research within the hospital clinic. Written (electronic) parental consent was a requirement for participation, and then written (electronic) assent was obtained from all participants. Additional parental consent and participant assent were verified prior to the interview for the interview participants.
: There is no identifying information included in this paper that requires additional consent. The participants were made aware through the information letter that “The results of the study may be published in an academic paper and in a report to the clinic, but you will not be identified”.
