: Written informed consent was obtained from each survey participant as well as the community leader who permitted project implementation in his community. The ethical commission of the Erfurt University and the Sierra Leone Ethics and Scientific Review Committee approved this study. The former suggested one minor change in the informed consent sheet, the latter none. All our procedures have been performed in accordance with the ethical standards as laid down in the 1964 Declaration of Helsinki and its later amendments.
