Data were de-identified and comply with the Health Insurance Portability and Accountability Act and the 1964 Helsinki Declaration and its later amendments or comparable ethical standards. The use of the IBM® Marketscan® and Optum® databases was reviewed by the New England Institution Review Board (IRB) and was determined to be exempt from broad IRB approval, as this research project did not involve human subject research.
