: Informed consent was not obtained for this study. All activities occurred using data from electronic health records or administrative claims that were originally obtained via routine healthcare operations. Each site submitted a protocol to their respective Institutional Review Board (IRB) – Vanderbilt, the Children’s Hospital of Philadelphia, Cincinnati Children’s Hospital Medical Center, and the Duke University Health System. The sites participating in this pilot query either received a waiver of consent or had the study found to be exempt (n = 2), or the IRB found the project to be non-human subjects research (n = 2). The IRBs of the teams that support the PCORnet Coordinating Center, Duke University Health System and Harvard Pilgrim Health Care also reviewed this project and found the Coordinating Center activities to be non-human subjects research.
