: Permission to carry out the study was obtained from the Ethics Review Committee, Post-graduate Institute of Medicine, University of Colombo. Verbal informed consent was obtained from parents of all children prior to recruitment of children in the study. In addition, assent was obtained from all children who participated in the study.
