: This study involved data from two population-based cohorts: the Lifelines DEEP study and the Rotterdam Study. Both studies obtained ethics approval from the respective institutional and/or national research committees and followed the ethical principles of the 1964 Helsinki Declaration and its later amendments or comparable ethical standards. The Lifelines DEEP study was approved by the ethics committee of the University Medical Center Groningen (document number METC UMCG LLDEEP: M12.113965). The Rotterdam Study was approved by the Medical Ethics Committee of the Erasmus MC (registration number MEC 02.1015) and by the Dutch Ministry of Health, Welfare and Sport (Population Screening Act WBO, license number 1071272-159521-PG). All participants in both studies signed an informed consent form prior to study enrollment and agreed to have their information obtained from treating physicians.
