: Ethics approval of the EMPHASIS study for the follow-up of the children in MMNP (“SARAS KIDS”) was obtained from the Intersystem Biomedica Ethics Committee, Mumbai on 31 May 2013 (serial no. ISBEC/NR-54/KM/JVJ/2013). Ethics approval for the EMPHASIS study in The Gambia was obtained from the joint Gambia Government/MRC Unit The Gambia's Ethics Committee on 19 October 2015 (serial no. SCC 1441). The EMPHASIS study is registered as ISRCTN14266771. Signed informed consent was obtained from parents and verbal assent from the children.
