Ethics approval and consent to participate

: Due to the study design being a case report, no ethics approval was formally obtained but all correspondence was still carried out using the principles of ethical conduct as stipulated by the University of Cape Town Ethics Committee.

: Verbal and written informed consent was obtained from both the patient’s legal guardians for publication of this case report and any accompanying images. A copy of the written consent is available for review by the Editor-in-Chief of this journal.
