Authors confirm that all methods were performed in accordance with the ethical standards as laid down in the Declaration of Helsinki and its later amendments or comparable ethical standards. The review board for ethics approval waived the need for written consent by the participants. Nevertheless, at each workshop, we declared that we were recording the discussion for qualitative research; we emphasized that the analysis and the report would be performed anonymously without mention of the name of the institution. We asked whether participants had an objection and if so, to request to have their remarks deleted from the study.
