Ethics approval and consent to participate

: Ethical approval was obtained from the Ethical Review Committee of Wollo University, College of Medicine and Health Sciences. Authorization letter was also obtained from CSA for downloading the EDHS data set by requesting the website ExternalRef removed. The data was used only for this study and it was not passed to other researchers. All data were treated as confidential and no need to identify any household or individual respondent interviewed in the survey. The detailed information on methodology and the ethical issue was published in the EDHS report.
