: This study was undertaken in compliance with the Research Ethics Guidelines of JICA ORI. As a result of the screening assessment, this study was exempt from an ethics review by the Research Ethics Committee of ORI. In undertaking the survey to households, surveyors obtained a consent to participate in the survey from every survey participant after explaining the purpose of the survey and the fact that all information provided in the survey would remain anonymous.
