: The trial was approved by the London School of Hygiene and Tropical Medicine Ethics Committee, The Regional Committee for Medical and Health Research Ethics, University of Cape Town Human Research Ethics Committee and from each local regulatory board before project commencement. This sub-study was approved by the Human Research and Ethics Committee of the University of Cape Town (HREC/REF: 092/2019). Written informed consent and assent were provided by guardians and participants under 18 years, respectively. Older participants (≥ 18 years) consented independently at enrolment.
