: ERES ethics committee approved the protocol (I.R.B No. 00005948). Permission to conduct the study was granted by FHI360 Zambia, Ministry of Health (MoH) and the district authorities in charge of ART facilities. All respondents in the survey provided individual written informed consent and the study was undertaken in compliance with declaration of Helsinki and key tenets of good clinical practices. There is no risk of confidentiality, as the data cannot be linked to the participating patients.
