Ethics approval and consent to participate
: Ethical clearance of the health economic evaluation was obtained by the Ethical Committee of the Medical Faculty of the University of Heidelberg, Germany (S-318/2009, 18th November 2009, Additional file ) and the International Review Board at the Ministry of Public Health in Kabul, Afghanistan (23rd January 2010, Additional file ). The trial was registered online at(ID: NCT00996463, 15th October 2009). Consort statements and flow chart are published in Stahl et al. []. An informed consent was obtained from all participants in the trial and documented digitally.
