: Ethical clearance was obtained from Addis Ababa University (AAU), college of health sciences, school of public health ethical review committee and Addis Ababa regional health bureau institutional review board (IRB). Official letter was submitted to the five hospitals administrative office and diabetes center coordinator. An informed written consent was obtained from each study subject aged greater than 18 years and verbal assent for those aged less than 18 years prior to data collection process.
