: All procedures were conducted according to the Helsinki declaration and the project was approved by the Norwegian Regional Committee for Medical Research Ethics (REK nr. 126065) and was conducted according to their recommendations. The participants signed a consent form before participating in the interview. They were informed verbally and in writing about the study, its purposes for publication, and the possibility to withdraw their consent at any given time before, during and after the conduction of the interview. Anonymity was ensured by changing all possibly identifying names, geographic details and personal characteristics.
