: Upon advice from the institutional ethics committee, formal review from the institutional ethics committee was not sought due to the retrospective nature of the study, the anonymity of the clinical cases that were the subject of consultation. It was impossible to seek informed consent from consultees retrospectively. The methodology of the study was therefore considered to be consistent with the principles outlined in the Declaration of Helsinki, and in line with the institutional definitions of a quality assurance project.
