: This project was been approved by the Institutional Review Board at ICES and the Research Ethics Board [REB # 12–012] at St. Michael’s Hospital, a site of Unity Health Toronto, Toronto, Ontario, Canada. Participants included in this study provided informed consent to participate. Under the Privacy Act, paragraph 8(2)(j), the CSC is permitted to compile personal health data for statistical use as long as the presentation of the information guarantees confidentiality and the information is used in a manner consistent with the purpose for which the data were collected.ICES is a prescribed entity under Ontario’s Personal Health Information Protection Act (PHIPA). Section 45 of PHIPA authorizes ICES to collect personal health information, without consent, for the purpose of analysis or compiling statistical information with respect to the management of, evaluation or monitoring of, the allocation of resources to or planning for all or part of the health system. Projects that use data collected by ICES under Sect. 45 of PHIPA, and use no other data, are exempt from REB review.
