Ethics approval and consent to participate

: The Regional Committee for Medical and Health Research Ethics (REC) in Norway, approved the study in the autumn of 2012, study number 2012/1460.A written explanation of the study and an informed consent form were delivered to participants and explanations were provided over the telephone. Additionally, all participants gave written informed consent to participate in this study. The study was performed in accordance with the Declaration of Helsinki.

: It is not applicable because the manuscript does not include personal details, images or videos. But, within the written consent form, there is statement which states that the responses of participants will be used for academic purposes.
