Ethics approval and consent to participate
: Written consent to participate was obtained from all participants and/or their parent or legal guardian. Norwegian health legislation states that that people between 16 and 18 may consent to health research on their own behalf. Individuals aged 12–15 may consent on their own behalf when the participant, for reasons that should be respected, does not want that their parents, legal guardians or child welfare services to gain access to this information. The study was approved by the Norwegian Regional Ethics Committee (2016/172, REK Nord).
