: The study was registered with the Danish Data Protection Agency (j.no. RHP-2017-009). The IMR and IPS studies are registered with the Danish Ethics Committee (j.no. H-1-2010 and S-2015-2000-163). According to Danish Law, no ethical approval from the Ethics Committee was required for the present study because no biomedical intervention took place.
