Ethics approval and consent to participate: This study was conducted with observance of the Declaration of Helsinki and the National Ethical Guidelines in Biomedical Research in Iran. As well, the study protocol was approved by the Ethics Committee of Shahrekord University of Medical Sciences, Shahrekord, Iran (IR.SKUMS.REC 1394.286 and IR. SKUMS.1398.070) at regional and national scales. All participants provided signed and fingerprinted informed written consent according to the Guidelines enforced by the Ethics Committee of the SKUMS. Before the data collection, we provided informed consent to all parents participated in the study, which mainly included a brief description of the study content and purpose. Participation in the study was completely voluntary and participants could quit at any time. Any information provided by the participants will be kept strictly confidential and used only for research purposes.
