Ethics approval and consent to participate: The study was approved by Regional Committee for Medical and Health Research Ethics, Western Norway (REK Vest), an independent public body established under the Norwegian Health Research Act. The committee’s secretariat is administratively hosted by the Faculty of Medicine, University of Bergen, Norway. Reference number: 681962. Participants provided written informed consent to participate in accordance with the Declaration of Helsinki. We tried to keep data collection as anonymous as possible by asking participants to give a nickname to be able to withdraw from the study. Quotations have pseudonyms to maintain readability.
