Ethics approval and consent to participate
: Written informed consent was obtained from all individual participants before inclusion into the study. The study has been approved by the responsible local ethics committee: Ethikkommission des Fachbereichs Medizin, Universitätsklinikum der Goethe-Universität, Frankfurt am Main, Germany under the committee’s reference number: 397/11.
