: This study was deemed IRB exempt by Oregon Health and Science University and Oregon State University. The need for ethics approval was waived and deemed unnecessary by the institutional IRB previously mentioned. Patients were asked if they would like to meet with the pharmacist and participate in the study beforehand. This was determined to be quality improvement and therefore no documentation was needed to show consent to participate, as this initiative is not outside of anything pharmacists usually do at the family medicine was part of their job.
