Ethics approval and consent to participate
: This study protocol and both English and Bengali version of the written consent form were reviewed and approved by the Research Review Committee and the Ethical Review Committeeof icddr,b (PR-16053). We also received approval from the Directorate General of Drug Administration (DGDA) Bangladesh, and by Oxford University Tropical Research Ethics Committee (OxTREC, Reference: 31-15) prior to data collection. Written and verbal informed consent were obtained from all informants and each participant was provided with a signed copy of the informed consent form. Study participation was voluntary, confidentiality was maintained, and participants were informed that they were free to discontinue from the study at any time without any consequence. In addition informed consent was provided by the photographed individuals to have their photograph taken and included in this report.
