Ethics approval and consent to participate

: According to Danish legislation, no ethical approval could be obtained for this kind of study (studies that do not involve human biological material) []. However, interviewees gave their informed consent either in writing or orally. If the interviewee wished to keep specific statements off the record, these statements were excluded. Interviewees are anonymous in the study and are referred to as the organization they represented.
