: This study was approved by the Research Ethics Committee of Makerere University School of Social Sciences, College of Humanities and Social Sciences. A formal letter was written by the Ministry of Health granting permission to conduct the study in the sampled health facilities. In each district, permission to conduct the study was provided the District Health Officer. For confidentiality and ethical purposes, the names of health facilities from which information obtained were recorded and analyzed using unique identifying codes.
