Ethics approval and consent to participate

: The Ghana Health Service Ethical Review Committee and the Institutional Review Board of ICF International reviewed and approved the GDHS survey protocol, including biomarker collection. Participants of the survey eachsigned a written consent form. The dataset used for the current study was anonymous, and with permission requested from ExternalRef removed only for the purposes of secondary analyses as was done in the current study.
