: All research was conducted according to the ethical standards outlined by the 1964 Declaration of Helsinki and its later amendments or comparable ethical standards. This study was part of a larger quality improvement project (i.e., Veteran-Informed Safety Intervention & Outreach Network [VISION]; see Houtsma et al. for more details), which was deemed exempt from institutional review by the Southeast Louisiana Veterans Health Care System Institutional Review Board. Participants were informed that survey completion and participation in a follow-up interview were voluntary. Verbal informed consent was obtained prior to interviews.
