Ethics approval and consent to participate
: Ethics approval was obtained from the Research and Ethics committee at the Aga khan University Hospital.Participants were recruited after obtaining written informed consent as per study protocol.Patients had the right to refuse or withdraw from the study at any point and this did not impact on the quality of care received subsequently.
